A 5-step guide for solving the RCA question:

    Step 1:

Always clarify the problem (May involve asking all basic questions that increases your understanding of the problem)

    Step 2:

Ensure that your analysis of potential root causes is collectively exhaustive (do not leave any reason that you can think of to be too small or large for causing the issue)

    Step 3:

Always have predetermined checklist of these potential causes classified into groups (For ex: Internal causes, external causes and Industry/macro-factor causes. This ensures you don't miss out on exploring root causes of a particular type)

    Step 4:

Try to determine the cause present in the interviewer's mind. (Your potential root cause may not be something the interviewer want's to hear. Hence, focus on the inputs he is giving and be guided to the root cause he's expecting)

    Step 5:

Finally, Be Patient. (It might take several questions to arrive at the required potential cause. Keep focusing on the interviewer inputs and to reduce the redundant questions that you asked.
